Appointments

We schedule all appointments to accommodate each patient's needs. Call our office to schedule your appointment. Anyone age 17 and under must be accompanied by an adult.

New Patient Forms

For your convenience, you may download our patient registration forms. We would like for you to complete and bring them with you at your scheduled appointment. If for some reason you are unable to complete your paper work, please arrive 15-20 minutes early.

Cancellation Policy

We respect the fact that our patients expect to be seen at their scheduled appointment time and pride ourselves in choosing not to "double book", (emergencies only). So, if a patient fails to show or cancels, it hurts our productivity and forces us to consider "double booking". Since we are not comfortable with this idea, we impose a $50.00 charge for each patient and/or each appointment cancelled or failed without a 24 hour business day notice (weekends are not considered). Appointments scheduled for more than one patient on the same day or appointments scheduled for 2 or more hours, require a 5 business day notice. This $50.00 fee must be paid before another appointment can be scheduled. If a patient is a chronic "NO SHOW or CANCEL", we will accept them only as a walk-in patient during an open appointment time.

Financial Policy

Payment is due, in full, the day of service. If payment cannot be made in full, you must inform our office prior to your appointment time. If you have dental insurance, we will gladly file it for you. We make every attempt to verify your insurance, however, any copayments and/or deductible are due the day services are rendered. YOU ARE RESPONSIBLE FOR YOUR DENTAL INSURANCE AND KNOWLEDGE OF COVERAGE. Any changes in coverage need to be given to the front office staff PRIOR to your appointment. When presenting you, the patient, a treatment plan, our office can only ESTIMATE the amount your insurance company will pay, based on the information given to us over the telephone. This is a service we offer is NOT A GUARANTEE OF PAYMENT BY YOUR INSURANCE CARRIER. ACTUAL PAYMENT IS DETERMINED AT THE TIME THE CLAIM IS PROCESSED. You will be billed if a balance remains after your insurance has paid.

Insurance

We consider our relationship with you to be of primary importance and will always make our recommendations to you based on what we believe is the very best treatment for you regardless of your insurance coverage. As the patient, it is your responsibility to deal with your insurance company and your employer. We will assist in any way possible to maximize your dental benefits, but to re emphasize; we have no relationship or responsibility to your insurance company.
FACT #1 Dental Insurance is not meant to be a "PAY ALL", it is only meant to be an aid.
FACT#2 Many plans tell their insured that they will be covered "UP TO 80%" or "UP TO 100%". In spite of what you are told, we have found many plans cover 40% to 50% of an average fee. Some plans pay more, some pay less. The amount your plan pays is determined by the contribution you and your employer make to your dental plan. The smaller the contribution paid into the plan for "insurance", the less you will receive. It is your responsibility to advise us of your insurance coverage’s, restrictions and limitations.
FACT #3 It has been the experience of many dentists that some insurance companies tell their customers that "FEES ARE ABOVE THE USUAL AND CUSTOMARY FEES' rather than saying to them "OUR BENEFITS ARE LOW'. Remember you get back only what you and your employer put into your insurance coverage less the profits of the insurance company. In dealing with over 500 dental insurance plans, most plans do cover our fees.
FACT#4 Each plan utilized in our office has different percentages, deductible, maximums, procedures covered, and varying fees that the plan will allow. We will do our very best to make as close a calculation as possible of what your insurance plan will cover. However, as we cannot estimate precisely, there may be variances for which the patient is individually responsible.
FACT#5 Many routine dental services are NOT covered by insurance carriers. We make our recommendations based on YOUR needs and not on what your insurance may or may not cover.
Please do not hesitate to ask us any questions about our office policies. We want you to be comfortable in dealing with these matters and we urge you to consult us if you have any questions regarding our services and/or fees. We will file insurance forms at no charge. We will do all we can to assure you of your maximum benefits.

We accept all PPOs and Private Insurance. Any insurance plans not listed above, we are considered to be out of network. We do not except HMO's, DMO's, or Dental Fee Plans.
